Postoperative cognitive dysfunction after cardiac surgery

چکیده
Postoperative cognitive dysfunction (POCD) remains a serious complication with increasing importance. Cerebral dysfunction after surgery presents as stroke, delirium, or POCD. POCD is the post-operative disturbance of memory, and speed of information processing. The incidence of POCD is dependent on the type of procedure. The highest incidence of POCD occurs in patients undergoing cardiac surgery. CABG performed without CPB could reduce the incidence of postoperative neurological disturbance. However, there were no distinct differences in the incidence of POCD between off-pump and on-pump CABG. Many risk factors including aging and poor cognitive status about POCD have been reported. The recent reports suggest that low cerebral oxygenation, as evaluated by near-infrared spectroscopy, during the surgery is a good predictor of POCD. The careful monitoring of the central nervous system during cardiac surgery might predict POCD, and help in avoiding its occurrence.

One of the incapacitating postoperative morbidities that was seen in all types of surgery is Postoperative cognitive dysfunction (POCD). The elder patients and also cardiac surgery patients (CSPs) are at particular risk. Additionally it was shown that the severity, extension of impairment and the incidence rate of POCD is more in CSPs. POCD is a dementia-like symptoms disorder is characterized ...

BACKGROUND Neuropsychological dysfunction is a major cause of morbidity and mortality after cardiac surgery. AIM To evaluate if intraoperative cerebral desaturation and depth of anesthesia measured by bispectral index are related to postoperative cognitive dysfunction in cardiac surgery. MATERIAL AND METHODS Prospective study in patients undergoing elective cardiac surgery with cardiopulmon...

INTRODUCTION Neurological and neurocognitive dysfunction occurs frequently in the large number of increasingly elderly patients undergoing cardiac surgery every year. Perioperative cognitive deficits have been shown to persist after discharge and up to several years after surgery. More importantly, perioperative cognitive decline is predictive of long-term cognitive dysfunction, reduced quality...
متن کاملPostoperative hyperthermia is associated with cognitive dysfunction after coronary artery bypass graft surgery.
BACKGROUND AND PURPOSE Temperature is a well-known modulator of experimental cerebral injury. We hypothesized that hyperthermia would be associated with a worsened cognitive outcome after coronary artery bypass graft surgery (CABG). METHODS Three hundred consenting patients undergoing cardiopulmonary bypass for CABG had hourly postoperative temperatures recorded. The degree of postoperative h...

BACKGROUND Remote ischemic preconditioning (RIPC) has been shown to enhance the tolerance of remote organs to cope with a subsequent ischemic event. We hypothesized that RIPC reduces postoperative neurocognitive dysfunction (POCD) in patients undergoing complex cardiac surgery. METHODS We conducted a prospective, randomized, double-blind, controlled trial including 180 adult patients undergoi...
